Completing this form does not represent a binding commitment to enrol, either for you or for the school. It is simply a good starting point: you can provide and receive all the key information in one place, and we can collect what is needed. This allows us to spend more quality time discussing your child’s educational path. And if online forms are not your favourite activity, everything can also be arranged in person at the school.

To begin, one note regarding the legal procedure: regardless of whether you choose a private or public primary school, parents must, no later than 15 March 2026, first register their child at the public primary school assigned according to their place of residence and complete the required medical examination with the designated school physician. Even if you have not yet completed these steps, nothing prevents you from continuing with this application. So, let’s proceed.

STEP 2 – Application for School Readiness Assessment

Although your child will complete the initial assessment at the public primary school assigned according to your place of residence, prior to enrolment at Prizma we would also like the opportunity to meet your child through a short and pleasant conversation with our school counsellor. The meeting lasts approximately 30 minutes and allows us to get to know the child in a calm and supportive setting.
